Probable Cause

DTSB: The DTSB determines the Probable Cause of this accident to be Pilot Error, resulting from the the pilots lack of rehearsal and conditioning in allowing the jet to accelerate past both rotation speed and a predetermined and briefed abort point using the DTSB 50/70 rule. The pilots late reaction and decision to stop beyond a point of being able to stop was fatal in this accident.

Recommendation

DTSB: The DTSB recommends that all pilots of all aircraft rehearse and condition themselves to become familiar with the 50/70 rule, and are more conditioned and prepared on each takeoff for an unexpected abnormal event during takeoff. The DTSB recommends that pilots plan for and brief an RTO scenario prior to each takeoff.
